Equipment site safety improvements and powered industrial truck accident investigations and reporting. 

At 1:53 p.m. on September 6, 2019, an Employee was moving a 3,200 lb. bag of cement, using a 2014 Toyota Forklift (Model: 8FDU30 and Serial Number: 6052).

 

While backing-up, the forklift then shifted to the left, tipped over onto its side, and the Employee was then struck by a metal guardrail. The Employee sustained a fractured skull and was killed.

At 11:17 p.m. on November 5, 2019, an employee was working for a temporary staffing agency. He had been sent to work in a warehouse. He was operating a Crown order-picker. While he was elevated to approximately 3 meters (10 feet), he backed the equipment with products on the picking deck. The boxes struck a racking stanchion and shifted into the operator station, striking the employee. He was crushed between a box and the operator cab. He was killed.

NON-OPERATOR TRAINING

A frequent question we hear from clients is, "Where's the value in training non-operators"?  The answer is quite simple, everyone is responsible for safety.  The challenge comes when employees workaround and interact with powered industrial trucks.

Many employee have no experience working around fork trucks and lift equipment.  So, they are unaware of obstructions to visibility, turning from the rear, turn radius, load obstructions, operators not driving safely ad more.

Employee turnover is also a frequent challenge as employees come and go their institutional knowledge of woring around equipment is lost.  

Another challenge is the ineffective leadership response to unsafe acts or activities they witness.  The tendency to look the other way is powerful with tight, demanding production demands and schedules.

Proper staff training is an important core component of any workplace safety program for powered industrial fork trucks.
