At approximately 5:00 p.m. on February 22, 2019, an employee was operating a stand-up reach truck-PIV. The employee tried to enter through an open bay door where the clearance was about 2 feet lower than the PIV height. At some point, the PIV overturned. The employee fractured his foot and was later amputated as a result of the injury. The employee was hospitalized.

If a workplace incident takes place, employers have a responsibility to investigate to identify the cause and hazards, which helps prevent similar incidents from happening in the future. This video shows the steps in an incident investigation and demonstrates the Employer Incident Investigation Report (EIIR) documentation and submission process. In this video, a supervisor and worker discuss an incident at work, how they corrected the issue immediately, and how they fill out their employer incident investigation report.
